Hole in the head, or HITH, is also known as head and lateral line erosion, or HLLE.It affects the sensory organs in the face as well as those along the lateral lines, causing pitting in both regions. There are four theories as to what causes HITH they are environment (water conditions and stess), diet (the most accepted theory), and the use of carbon in filtration (more specifically the dust from carbon)
Treatment
Feed a balanced and vitamin enriched diet including veggies and fruits. Use a vitamin supplement and soak food in garlic juice.
Do water changes. It is important to keep the levels at ammonia 0ppm, nitrite 0ppm, and Nitrate below 10ppm.
Add 1 tbs of aquarium salt to every 5 gallons of water. If you have any scaless fish in the tank such as pleco's use a dosage of one tbs per 10 gallons. When a water change is done replace only the amount of salt for the amount of water removed. Pre-disolve the salt in tank water and add slowly over several hours.
Raise the temperature in the tank slowly to 80-82°f
Remove all carbon from your filters
Hole in the head is easy to cure but takes quite a bit of time to cure do not expect results overnight.
